Hi. I did long haul with my son in nov, he was the same age. For the carry on: definately lots of nappies (though think they have 'emergency spares'), baby food in jars (they often make you sample some at customs), plenty of wipes.muslins etc. several changes of clothes for bubs and for you (at least ahange of top)
Dummies (attached to bubs by cord/string etc and safety pin as you dont want it falling/being thrown on the floor all the time)
for the time in wales: Buggy/carseat/portcot. We didnt take these, instead hired or borrowed from people over there, much easier I found. We took a 'me-too' phil and teds clip on high chair which is very comapct and was great.
this time of year over there is much like it is here at the moment weather wise, though should be getting warmer not colder. As long a you have a hat for him and a good coat/woollen jumper for outdoor you should be OK, the key is layering. I found merino excellent.

I did long haul with my son last year (he was 3-4 months old then). I found that I actually ended up taking too much of some stuff and not enough of others. I think I went overboard on bibs etc and it got a bit stressful at times when I was trying to find things in my bag but there were so many bibs etc to rumage through!!
I must dressed my DS in sleepsuits and brought a few spares - easier I thought than lots of changes of clothes.
Perhaps you are still BF so that will be easier but the airline crew are great at heating up any formula you might need - just call them well in advance.
If you can fit it in, see if you can bring a thickish softish mat/cloth for nappy changes. My DS screamed the place down because the change table in the plane is so hard!
As Wellymummy said, make sure and pack something in your carry on that is warm for bubs to wear when you arrive at airport in UK as it could be cold.
I found a front pack/sling an essential as I was on my own and needed my hands free.
