I will be returning to work at the beginning of June and am required to give at least 1 months notice of my intention to do so. I know its still early but I like to be organised, and its all a bit scary TBH.

I have a pretty good relationship with my manager and I have already had an unofficial catch up with her re hours and days. I have started writing a letter and so far have this:

Dear ........

Re: Return to work from Maternity Leave

In relation to my Maternity Leave, I wish to notify you that I intend to return to work from maternity leave. I would like to return to work on Monday 7th June 2010.

As previously discussed of my return to work, I wish to work permanent part time 4 days per week. The days I would like to work are Monday, Tuesday, Wednesday and Thursday and from 8.30am to 4.30pm as these are the days and times that fit in with our child care arrangements.

Is there another information that I should or need to include?

TIA

Sounds good to me. If you're on good terms with your manager, they probably just need it to put in a file somewhere
