So I am seriously considering opting for a CS after having a 3rd degree tear and forcep delivery with DD. While I healed quite well, things have never been 100% back to normal in terms of bladder control etc. It's not bad, just not as it used to be IYKWIM.
I've talked to my MW about it and she comes across as pushing me towards delivering naturally. Although I'm sure she would be supportive if I wanted a CS. I just don't want to tear again and make things worse and have to have surgery anyway.
She said I'll be referred to hospital at 20 weeks to talk to them up there about the birth.
I also had high BP which is more than likely to happen again. THey induced me on my due date becuase of it and things went really slow. For me, the whole being induced was not a nice experience at all.
Has anyone else been in this situation and did the Dr's recommend a natural birth, or did they give you the option of a CS?

I haven't been in this situation but my sister had a 2nd degree tear and she said that delivering her next baby naturally (5yrs later) 'fixed' her down there...no tears and now it's better than it was after the first birth. I know yours is 3rd degree but just thought I'd give you that info...
I was induced, had a 2nd degree tear and ventouse delivery and while the thought of a natural birth worries me a little bit I keep thinking about what my sister said...you don't often hear the good stories...

I have been where you are i had a episiotomy(sp) forceps delivery then hemorrhaged. My recovery was months, much longer than a c sec would have been and sometimes i do think that would have been easier.
But it would never choose to have a c sec over a natural birth.
Ive done quite a bit of research on this and chances are low that i would have a repeat birth 2nd time around.
I think you need to do a bit of research and having a good midwife will help too.
There are lots of things you can try to prevent tearing as well. Ill try find some of my info and post it up

I don't know about the being induced side of things, but I had a 3rd degree tear with #1 (with ventouse delivery) and was really scared about having another tear for #2.....But I ended up having a water birth with no drug and NO tear For me the main difference was not having the epi and trusting in my MW (it was REALLY hard not to push when she told me to stop, but in the end thats what made the difference).

I posted a thread similar to this a few weeks ago. I had complications from pushing for way too long/forceps delivery resulting in anal damage. It was horrendous the first 6 months after birth and although much better now, I still suffer with relapses every now and then.
The thought of doing serious damage with life long consequences scares the life out of me. I also wonder if an elective CS will be the best option for me next time around.
Sorry, I'm no help, but I feel for you cause the idea of a natural delivery after what you have been through must be pretty freaky.
I will be interested to hear what the hospital says.

i think its important to research and discuss the risks associated with both... there is risk of damage to your bladder and bowel with a c-section, I actually remember the OB saying to me when i was signing my release form with Ollie "there maybe damage to your bladder or bowel however will do our best to fix it"
Not trying to freak you out but just wanted to point out that there are risks associated with sections that most of us would never even contemplate so best to weigh it up with experts.
Having said that, i have known of women who have been recommended to have an elective due to the damage done during labour so its definately possible!

My SIL had a bad 3rd degree tear with her first, recovered well though. She had her 2nd naturally and it was a completely different experience.

There are risks both ways, and the risks are roughly weigh out the same.
Definitely do you reasearch, you have heaps of time to make a decision hun.
Ive never had a baby vaginally, first was emerg CS and second was elective, and my elective CS was great, I loved the experience, it was an aweosme birth compaired to my first. I had no issues after having DD and i was healed up and walking rounf properly really soon.
You could PM Aroahnui(Liz) as she had a 3rd degree tear with her first and an elective CS for her second.

I didn't have a 3rd degree tear but a 2nd degree tear and a large internal tear. That was with my son. I was also induced (both times) but that was from labour not progressing properly after my waters broke.
It was a pretty horrible labour and birth. I hemorrhaged and lost 2 litres of blood. Those were the main issues, the tearing and blood loss. It was a ventouse delivery.
I was so worried that things would turn out the same next time. But they didn't. Yes i had to be induced again (synto both times) but it was a way better birth. No complications this time, just a couple of grazes. It was also way shorter and WAY less pushing time. Had epidural with #1 and just gas with #2.
Well that probably doesn't help you but thought i'd share my experience anyway. lol.
Good luck with whichever way you end up birthing.

Like many others have above i had a 3rd degree tear with my first boy - very tight perineum apparently. So next time my midwife and i talked about the ways you can prevent tearing - like not pushing the baby out too hard, kind of controlling the delivery a bit more, they can put some reverse pressure on the head to slow the delivery down, water labouring (not birthing tho) to soften the perinuem.
TBH your second time around you do feel a lot more in control as you don't have the fear of the first time, it's all more familiar. Hard to explain but you just feel more - well yeah in control i guess.
So anyway we did all that and when crunch time came she said no way your perineum is really blanched and you will tear so she gave me an episiotomy and out came baby. So that was better because she controlled the angle and degree of tear so then it doesn't involve the anal sphincter. Actually like someone else said my bladder control has felt better after the second time!
I've never had a cs so can't compare but second time natural labour with no pain relief went really well, quite quick and easy.

I had a 3rd degree tear(30+ stitches internal and external) with my first. I am much the same as you. Bladder control is crap. I've actually even retorn the scar tissue during intercourse. i chose to have a c/s the next time and it was the best decision I could've made for me. I was so terrified about tearing again. Not only for the damage it causes but I remember feeling that tear like it was yesterday and never want to feel it again. My c/s was so great. I knew what was going on at all times. The nurses were incredible. I went home after 3 days and was all good and driving again in 3 weeks. It was a totally positive experience. Bfeeding went great. Certainly wasn't affected by the medications. I think it was really about 3 months before I felt completely normal again. Which was similar to the recovery after my tear.
Everyone is totally different and there are risks either way you go. Hope this helps though :)

First of it's different for everyone, so you really need to discuss options with MW and OB etc. And at the end of the day you need to be happy with the decision.
With my first I had a severe 3rd degree tear (60+) stitches....... took quite some time to heal.
With number 2, I had the same delima as you.... and decided that for me I would go natural because there wasn't a guarentee of a tear..... while in labour I freaked out somewhat about tearing and actually felt the tear...... ended up with a bad 2nd degree (30+) stiches....... healed much better this time, and although still not perfect down there, the postitives for me were that I could still lift and play with my toddler whereas with CS you can't for 6 weeks.
And now I'm facing the same delima being pregnant with number 3. I'm 98% certain that I'll be going natural again this time and am hoping for a waterbirth....
So thats just my experience. And like I say you might tear again, you might not.... you just need to weigh up the pros and cons of all the options and make the best decision for you.

i haven't given birth naturally so can't really comment. I have had an elective C Section though because DD was breech.
I felt the C Section (though i didn't have a choice) was a very positive experience. I was mobile 8 hours after surgery, showering etc.. I didn't have any probs doing any things i needed to do for you new baby and had a speady recovery. was home on day 3 after surgry and feeling almost 100% two weeks after CS.
Keep in mind that you MW will not get full pay if you have a CS so that could be influencing her giving you the natural advice.
I had a friend you had her baby 1 day before i had my CS. She had a natural birth which took 20 hours, large 3 degree tear and forceps etc. She was in hospital 3 days longer than i was and seemed much less mobile and in more pain than i was after my CS.
Elective CS can be very positive experiences (like mine). There are risk associated with it tho.
I will happily opt for a CS next time if i get the option.

I could have written this post, though I don't have a number two yet. I am terrified of tearing again, but there's no way I would choose a c-sec over a natural birth, and I am convinced I tore so badly because of my epi and because there were so many people in the delivery room watching me push so when my MW told me to stop pushing, other people were too excited to hear her and kept cheering me on!

I have just had my baby girl 3 months ago and had a third degree tear as well as a hemeatoma somewhere down there that they had to drain and stitch. wasn't pleasent although I have healed really well.
I tore because my baby was huge (11lbs +)  and have been reccommended to have a cs next birth. Mostly due to potential size though and the probability of tearing again and not healing as well.
I am quite happy with that...not really keen to push out another 11lber, but I think if my baby had been smaller and the chances that the next baby would be small as well, I would probably opt for natural if poss.

You do have ages to make the decision, so don't feel like you have to rush into anything, GL

Thanks everyone, some really good perspectives from both sides.
I think the plan is to see what my BP does and also the size of the baby. MW is happy with that. I'm not keen to be induced again, and if I have high BP again then I'm bound to be induced if I don't go naturally before due date.
Anyway, as you say JoJames, heaps of time and will be keen to hear what the Dr's say at the 20 week referral.
Does anyone know how 'late' you can elect a CS, can you elect one say in the 38th/39th week??

They only found out DD was in breech Position when i was 38 weeks and i was booked in for the next weeek.
They like to do elective CS by 39 weeks. So i would guess you would have to opt in well before that to ensure they can fit you in. I think they would only squezz a CS in for someone 38 weeks gone if there was a medical reason like breech presention
