I've had a c-section as my son was too big to fit, and I'm hoping if I ever get pregnant again that I'll get a chance at a VBAC. My mother had three kids - her first was a c-section, then she had a 'normal' birth, then last (me) was another c-section (too big again - my sister was the smallest and only just came out with forceps etc). Since you had one for breech, there isn't any known reason you can't get a normally-aligned baby out safely, so I can't see any issue with you getting a VBAC! I was lucky with my c-section that due to my family history (having the same body-type as my mother with absolutely no waist height) it's wasn't a surprise for me, but I'd dearly like to get lucky with a smaller child (DS was 9.5 lb).

    Author: Laurz_20
Subject: 33868
Posted: 19 June 2013 at 10:03pm

Just wanted to say hi Marama and tell you about my recent experience (DS is 3 weeks old). I had an elective Cs with DD because of polyhydrominos and a large baby she was 9lb 11oz. I was really determined to get my VBAC so I found what I thought was a great midwife but she wasn't very supportive and said I wouldn't have a choice re continuous monitoring which I was not keen on and strongly recommended an epidural. At 32 weeks I decided to find a midwife that was more supportive of my choices. I found a fantastic supportive midwife who I quickly came to trust. Anyway about the labour...MW broke my waters as I was 5cm and in no pain. Requested to labour in the pool and I was able to, I was monitored via the Doppler every 10-15 mins never was strapped to the bed. Had gas only as pain relief which was all I wanted. Bub was born after 2.5 hours in the pool was an amazing experience and so happy to get my ideal birth, bub was perfect at birth, a very healthy 9lb 6oz and scored 10, 10 and 10 on his apgars. Of course I always would have done whatever was needed to keep my baby safe which I did do with the full backing of my MW and OB. If I was to give any advice I would say stick to ur guns, ask lots of questions and make sure u have a supportive midwife. All the best Marama:)

We had a long chat about continuous monitoring as that was the only sticking point, and she said that she would feel more comfortable with cm but it was up to me and in the end her desire to give us the birth experience we wanted overode her preferences. So that's all I need for now, just to know that there is some wiggle room.

fadeless I know that my mw has attended 6 VBACs this year and 5 were successful so I thought that was a pretty good success rate. Haven't asked her about the hospital VBAC rate, will do that when I see her next week.

Thanks Emmi yeah I found that doc too - just cs rates.

It seems pretty quiet on OB these days compared to a few years ago!

Edited by Marama - 12 June 2013 at 2:47pm

She was born severely anaemic and it's a mystery as to how it happened, most likely scenario is a slow foetal-maternal haemorrhage (ie bleeding thru the placenta into my circulation). She spent 5 weeks in NICU and was lucky to survive, but is now a happy healthy little girl!

So we are seeing an obstetrician this time for extra tests and monitoring....I had my first appt with her last week and came away feeling overwhelmed and intimidated! She is extremely risk averse and pressed strongly for an elective CS at 38 weeks EVEN if the tests all come back fine! (She was the one who delivered DD and I think feels an extra duty of care because DD was so sick and it was completely missed).

When I told her I wanted a VBAC she told me all the risks etc then said if I was 'allowed' to labour then my labour would have to be textbook and not go on too long, otherwise she'll recommend emergency CS. Sheesh, no pressure!

Since then I have decided that I need to get informed and get into a positive, empowered mindset. I been doing lots of reading including this thread which I have found very helpful.

I am also interviewing a new midwife this week who is apparently pro-VBAC (so was the midwife we initially chose but now she has to go overseas for family reasons when I am due). So fingers crossed she works out!

OK, I'll leave it there and see if anyone wants to chat!

I had my wee man on 8th Feb and got my VBAC, I had a S&S at 1.30 on the 7th Feb at 39+3 and my MW said my cervix was well back and she didn't expect I'd be having my baby anytime soon. She did what she could and I left feeling a wee bit dissapointed. I had a few errands to run while my toddler was at preschool and felt a few wee pains, but just assumed that was from the S&S.

At 4.30 just as my partner walked in the door I got my first proper contraction, like my previous labour it all starts happening quite quickly with me, so I downloaded an app on my phone to time them as last time my partner was a bit useless at that job and found they were pretty regular and lasting 40-60 secs. shortly after 5 I phoned the MW she said she doubted it was labour just the body settling after the S&S and to give it an hour, we phoned MIL to come over and got my DD fed and ready for bed. I phoned the MW again just after 6, I knew it was definitely all go, hey I'd been here before (haha)... She said she'd come over and check, when she got here I was a good 3cm's and she suggested we make our way to the hospital where one of my other MW's would meet us just after 8.

On arrival at the hospital the contractions were progressing. They set me up in the birthing suite and got me sorted with some gas.. The MW came and did a check I think I was about 5-6cm at that stage, she said she'd do another check at 10.30/11, I'd been so insistent all along I didn't want continuous monitoring and here I was stuck on the bed, she said I could move around but I was so sore at this stage I didn't want to move, I did make the effort and had a walk around, loo stop etc and then climbed back on the bed on all fours leaning over the back of the bed. I asked to be checked again just after 10 and she decided to then break my waters, I immediately went to 8cm and from then on it was all go... The contractions were now coming thick and fast and before I knew it I wanted to push, but when I did push my CS scar really hurt, the OB came in and was happy with baby's HB and comfortable that I was at no risk of having a uterine rupture so decided to use the ventouse just to help me to push the baby out. I didn't need to push for too long before we had the head out and then the shoulders etc... My wee George William had arrived at 12.08am weighing 3.420kg... I did have a 2nd degree tear so was stitched and within a few hours was off to Burwood Maternity Hosp where I stayed for 2 nights. Unfortunately George ended up in hosp at 5 days old, but he is now 5.5 months and almost 8kg and just awesome!

I asked my MW about monitoring yesterday and if we could do an every 15/10 minutes type thing, but she said no, that it really had to be continuous. I mean I know why, because fetal bradycardia is an indicator of rupture, but if I had scar pain I'd certainly consent to continuous. I'll bring that up again sometimes after 30 weeks. Maybe I can get some info from the OBs on that at my appointment?

I'm hesitant to swap as I picked my MW for several reasons and VBAC was only one of them. I had such a ridiculous time finding a MW when I was 4 weeks pregnant, hate to think of what it would be like now.

My MW is at least happy for me to hang out at home for some time and I think I'll push how long I hang out past what I did last time (because I was only 3-4cm when I arrived before, I just started feeling I might want some gas or the pool, but as I'm not getting either of those I'll wait longer). I need a fair amount of postpartum support (baby will likely need to be referred to specialists and have surgery for a difficult tongue tie- another thing I have to argue my case for) which is more important than the VBAC and found that really hard to come by last time (got brushed off by everyone, MW, GP, Plunket etc).

I was at women's in 2010 and they had ctg ( belly band electric monitoring), and fetal scalp monitor and then a hand held like what mw uses at appts. I'm pretty sure they don't have wireless as I would be after for my upcoming both in jan if I could! I know welly hospital have one so not sure why Chch doesn't, would be great for us vbacers though. I really wanted to use the pool (with monitoring in there) but they said no way, presumably as there are no monitors for that.
I know what you mean with the mw as you would actually have to find a new one to get all those non standard care things. Last time I was set to use the pool and had the initial external monitoring via the belly band and that picked up bad decelerations, confirmed later by the internal monitor (and then heavy meconium and failure to progress enough given the decelerations and meconium = CS).
My first choice would be every 15-30 minutes or so, then something wireless and waterproof for constant monitoring, but I could be ok with just wireless. I'm planning a VBAC and though I'm a bit off from my OB consult I thought I'd ask if anyone had suggestions on what to ask my MW, in terms of what she'll support, and the OB.
Some of these are probably things I should have asked when I was interviewing my MW, though I know she's at least nominally VBAC supportive. I had a very difficult time finding a VBAC friendly MW at all for some reason, so after about a dozen different midwives it was like, 'oh you'll support a VBAC AND you seem ok so I choose you!'.

Here's what I have so far: what can I expect my VBAC labour to look like? Will I be allowed to labour in the pool, move around, be off the bed?
Which of the typical OB guidelines (coming to hospital when labour starts, IV line, constant monitoring) do you recommend I follow? What kind of monitors does the hospital have?(I entertained the idea of a HBAC, but as finding a VBAC MW was so hard I settled for that)
How involved will the OBs be when I am in labour?
Will there be a labour time limit?

Any other ideas? I've read most of the thread to see what others have said.

BUT that doesn't mean you have to be lying on your back. And like Haley1, My MW said 'continuous' meant regular rather than non-stop so were going to do it by hand every 2nd contraction once things got crunchy so I could be in a pool or whatever. Unfortunately I was quite early (34 weeks) so they wanted truly continuous monitoring (more for bubs than me) but I was still able to kneel, be on a Swiss ball, be on all 4's on the bed etc with the straps on (the straps being for the monitoring thingy). The MWknew I didn't want to be stuck on my back. Biggest down side for me was being able to hear bubs heartbeat almost stop during contractions so I knew he was getting stressed and worrying about that distracted me from pushing. Next time I would ask if they could turn it down so I could focus better.

Edited to make more sense.

Edited by thesaff

I am 14 weeks pregnant and desperately wanting a VBAC. My son is 15 months old and was an Emergency CS for breech presentation. My waters broke at 36 weeks and then at hospital they discovered he was breech, was booked to have an elective the next morning (am now quite annoyed they never let me attempt a VB) but I started contracting overnight but theatre was full of proper emergency CS's so had to wait until 9am when I was 6cm already. I hated been strapped to the bed being monitored, I think it made the pain much worse. The CS was fine (had skin to skin straight away) and I recovered really well but it was just not what I wanted and it was awful not being able to properly care for him on that first day.
My MW thinks there is no reason why I shouldn't have a straightforward VB this time so I am about to start all sorts of exercises to make sure this baby is not breech. I was later told my baby had a short (27cm) umbilical cord and that is most likely the reason he was never able to turn.
I have to deliver at middlemore as I am too high risk now to have my baby at the local maternity unit. A good friend of mine just had a successful VBAC there so I am hoping for the same result.

Congrats on the arrival of your baby girl Amy.

    Author: NZ-rules
Subject: 33868
Posted: 09 March 2012 at 9:01pm

Hey ladies!

Well, wee Freya is now almost 6 weeks old so I thought I'd better come in and let you know how the birth went (while I can still remember!).

I didn't get my VBAC, however I did have a really really good C-section so I'm not too worried. I went into labour naturally on the 30th of January with a few random contractions around dinner time, and they slowly built up to around ever 6-7 minutes around 10pm, then started to get pretty ouchy around 4am.

By 7am they were every 4 minutes and my MW asked me to go to hospital to be monitored. After a horrible trip to hosiptal in rush hour traffic, we arrived at hospital around 8.30am. I managed to get one of the wireless monitors which was fab, but unfortunately it showed that bub's heart rate was dropping with each contraction and taking longer and longer to come back up. Around 12pm I was stuck at 3-4cm so the registrar suggested my waters be broken (I agreed hoping it would move things along as I was I tired and sore by this point) and there was thick meconium in them.

An hour later there was no progress and the heart rate was getting lower and lower so it was decided that a c-section was the best option. 15 mins later I was on the operating table and bubs was out shortly after. The cord was around her neck twice and had been squeezed with each contraction which explained the heart decelerations. Once she was out they took an age to get her breathing as she'd inhaled meconium, and then she was still gasping for breath so they took her straight to NICU for some oxygen with me only getting a brief glance (it was sooo horrible). She ended up spending 36 hours in NICU, but she has no long lasting effects from her breathing issues.

I was up and walking the day after she was born and driving 2 weeks later, so my recovery has been fantastic, and most importantly my little girl (who is pretty darn gorgeous) arrived safely

Good luck to the rest of you waiting for your little ones to arrive - I hope you all get the births that you want

Alot of the info you get on the net is US based, I found a book that is UK based and has found it to be the best help in me making my decision it is Vaginal Birth after Ceserean by Helen Churchill and Wendy Savage, there are a few stories in there some straight forward births and some not, but all women achieved their VBAC.

Good luck in making your decision, I really hope I'll be on here in a few days to tell you about my successful VBAC

It ment I could roam around the room as I liked (I was only on the bed for internals!) and could hop in the shower or pool (but I didnt think I would like the feeling of the water, or of being wet so I didnt) but I had organised to have ht birth pool when I was all good for a HB, so again you must be allowed in the pool?
If you read the best practice guidelines for VBACs (ask your MW for a copy), it says that there is no imrpoved maternal or fetal outcome of continuious monitoring vs intermittent... so use that as ammo if you dont want it (I ended up taking the monitors off when I was in the full throws of labour, but it didnt make any difference to my uncomfortableness so didnt mind them being put back on)

Oohh Vix, not long to go!!! I will discuss these witn my mw wnen the time comes but wiuld like some facts so i know what questions to ask (if that makes any se se)

First of all, is continuous monitoring necessary? I want my baby to be safe but i want to be able to move around, as i feel that having to be constantly monitored last time, and not being able to move around did not help me at all.

Also, i really had my heart set on using the bath or shower for pain relief, maybe even having a water birth. But from reading this thread it seems that this is not allowed if you are having a vbac? Does anyone know why this is?

If bubs is in a good position then it might be no biggie, but polyhydramnios (too much fluid) can make it trickier to keep a good position with all that lovely fluid to swim around in!

Thank goodness I'm having a scan tomorrow to double check she is head down. The OB said nothing unexpected - lots of stats on uterine scar ruptures. I asked him how many he'd seen... apparently he'd 'heard' of 2.... He wanted me to book an elective at 10 days OD but I said I won't have one before 14 days which he was fine with. So if it's in a less-than-ideal position, it has the space to manoeuvre into a better position. With my second DD, she was posterior for most of labour, but because my waters were intact, neither my MW or I were particularly worried - although it did hurt to have such a long transition as we waited for bub to turn around. The relief when she did was awesome! My waters with her broke on my first push, they'd done their job.

With my first DD, my waters ruptured first of all, and although it was a hind leak, I lost most of the fluid in early labour. DD1 had her head on a less than ideal angle and so she didn't impact the cervix the way she should have and the contractions never really got co-ordinated. You can imagine being in a deflated balloon would be a lot harder for her to reposition herself as the balloon would be all clingy and squashy against her. Pretty much whatever position she was in when the waters went, that's how she was stuck.

Hopefully that balloon analogy makes sense?

The other bonus for me was that cos waters broke once I was pushing, they had moderate meconium in but a transfer was out of the question at that point cos bub was well on her way. She was born 11 minutes later anyway, so no time to worry about it either!

    Author: Emmi_
Subject: 33868
Posted: 03 January 2012 at 6:47pm

I just wanted them to break naturally, let labour follow its natural progression. Breaking your waters can make your contractions a lot more intense. Also with DD they started off my induction with breaking the waters (since I was already 3 cm), and her head was in a bad position, since I was being induced the contractions were just trying to force her poorly positioned head down, resulting in it swelling and not doing too much else, so I felt that if I left mine to go naturally then baby would have as much chance to have a good positioned head as possible (since once they are broken there isnt the room or fluid to help them reposition themselves, making it way harder to do so). I also didnt mind if baby was born in the caul (I think the next one will be, I doubt my waters will ever break naturally)
There is also the risk of cord prolapse with AROM, and infection (also why I didnt really want any VEs, especialy after waters had gone).
There are a few pages that come up if you do a google of riaks of AROM if you wanted to find out more. To me it was just another intervention that I didnt want if it wasnt necessary, but I am happy I had them done when I did, since nothing much was happening, and baby was still too high to do anything with
