Yeah I read the book, it was awesome! I couldn't find the book anywhere else either, so in the end I bought it online from fishpond.co.nz. You can try them.

I think the part that I found most useful was the illustrations of various definitions of CM. That is something that no one else can tell/show me, so the pics in the book explained a lot.
